Booking/Consent forms for Its My Life Festival 2026

Please can you ensure you have selected the correct day and correct cohort of young people. Each young person needs their own form.

We work on the basis that the person registering the young person onto any activity will first have made a judgement about the suitability of the activity for the young person. Any information received will be used to better understand the young person and ensure they have the best time at the festival.
