What is it about?

The public health team in Essex is working with the local NHS Clinical Commissioning Group to understand the health needs of residents living at Templefields House, Greenway House, and Terminus House in Harlow.

This project is seeking to understand the needs of residents, what health services are in place, and what gaps or barriers there are to accessing health services in Harlow.

 We would appreciate your help in giving us your views to help us improve our services.

What would you like me to do?

We would be grateful if you could complete this questionnaire by end of day on Friday 31st May.

This questionnaire is aimed at understanding your health needs and those living with you, what services you are accessing and any challenges you may be facing in accessing services.

Please complete one questionnaire per household. The questionnaire should be completed by an adult over 16 years of age.

This questionnaire is the same as the paper one that was posted to you. There is no need to complete both.
