Parent or guardian feedback survey

Overview

Listening is important to us.  By listening to what you tell us we can make sure you get the help and support you need.  By filling in this form and telling us what you think we can make sure that you and other children are well supported by social workers.  This short survey is an opportunity for you to have your say about your involvement with children’s social care services. It will help us ensure we are meeting your child’s needs. 

What you tell us is anonymous which means your name will not be mentioned and you will not be identified from our reports. Your feedback will be kept safe and will be shared with your social work team manager. It you tell us that you or your child is unsafe or in need of help, your social worker may contact you to talk about these things.

Under the Data Protection Act we have a legal duty to protect any information we collect from you.  This information will only be used for the purposes described above and will not be kept longer than is necessary to do so, paper questionnaires up to 1 year and electronic files up to 5 years. We will not share your personal details with another agency unless we have concerns that you or another individual is at risk of harm.
