Overview

Listening is important to us.  By listening to what you tell us we can make sure you get the help and support you need.  If you fill in this form and tell us what you think, we can make sure that you and other children are well supported by our social workers. 

What you say is anonymous which means your name will not be mentioned in our reports.  What you tell us will be kept safe and will be shared with your social worker and team manager. It you tell us that you are unsafe or in need of help, your social worker may contact you to talk about these things.

If you need help filling in this form, please ask an adult to help you.

Under the Data Protection Act we have a legal duty to protect any information we collect from you.  This information will only be used for the purposes described above and will not be kept longer than is necessary to do so, paper questionnaires up to 1 year and electronic files up to 5 years. We will not share your personal details with another agency unless we have concerns that you or another individual is at risk of harm.
