The Children and Young People with Disabilities Service: OT and Rehab Team are a specialist team who work with Children and Young People with permanent, long term disabilities in their home environments to enable them to access essential facilities, where unable to do so as a result of environmental barriers. We provide advice and information, manual handling guidance, loan equipment and provide adaptations where it is considered necessary and appropriate to enable function within the home setting.

This session is designed to provide an overview of our service including eligibility for assessment and how to make appropriate referrals to the team

This session will include a Q&A and you can ask your colleagues anything to do with their organisations, services and daily practices, to help you gain a better understanding and promote an improved collaborative working approach.
