Overview

Reflective practice provides the opportunities for those working with the adult/carer to retrospectively consider the impact and outcomes of their intervention(s) and drawing out learning for future practice.

Critical reflection is a purposeful and structured activity for thinking back on your interventions - so what took place, evaluating the impact of your actions, considering what went well, and changes/ improvements for future practice.  In doing so, this should enable conversations around quality of practice, achievements, and support required to facilitate good practice.

This reflective tool is part of the practice audit process and is linked to the audit for a specific case.  These reflections will be used to inform individual, team and service developments.
