Our Charter is a set of SIX promises we make to our care leavers. Please complete this short survey so that we can better understand your experiences and know whether the Charter is working for you.

What you tell us in this survey will be kept safe. You can choose to complete this form anonymously by not telling us your name or date of birth. Or you can provide this information and we will share your feedback with your personal adviser. If you tell us that you are unsafe or in need of any help then your personal adviser may contact you to talk about these things.

Under the Data Protection Act we have a legal duty to protect any information we collect from you. This information will only be used for the purposes outlined above. Your survey feedback will be kept electronically for up to 5 years.  We will not share your personal details unless we have concerns that you or another individual is at risk of harm.
